PHI should not be stored on a blockchain — it is impossible to delete, which conflicts with HIPAA's right-to-access and correction requirements. The correct architecture stores PHI in encrypted off-chain storage (under the covered entity's control) and stores only hashes, access permissions, and audit events on-chain. This provides blockchain's immutability and auditability benefits without storing PHI in an un-deletable record.
